What does this statistic mean, and how are municipalities preparing? This aging demographic will have an impact on all facets of our communities. To gain understanding of this aging phenomenon, the Metropolitan Area Agency on Aging (MAAA) conducted a survey of 146 metropolitan cities and townships to determine the status of their planning for an aging population. MAAA modeled their Communities for a Lifetime Survey after the National Association of Area Agencies on Aging's Maturing of America Study. The program is designed to reach elderly patients who do not have easy access to a medication review due to poor insurance coverage, a physical or mental disability and/or a debilitating chronic illness. The visit can be completed at no charge to the patient; however, donations to make the program available to other seniors are accepted and appreciated. Under the Fairview/MAAA program an MTM pharmacist is able to travel to the patient?s home, complete a medication review, communicate with the patient?s physician and follow up with the patient. Medication reviews look for the following: 1. There is a reason for a medication 2. The medication is working 3. The medication is safe for the patient 4. The medication is convenient (cost, administration, other focus) To be eligible, patients must be age 60 or older, take at least four medications, be homebound, and live within Anoka, Carver, Dakota, Hennepin, Ramsey, Scott or Washington Counties. The program has been in place for 3 years and patients are regularly scheduling appointments. MTM welcomes new referrals. If you know of clients, friends or family members who may qualify, have additional questions about the program, or would like to schedule a presentation, call Kristen Fischer at kfische5 at fairview.org or Amanda Brummel at ARHODE1 at Fairview.org. ------------------------ Bremer Emergency Fund The Bremer Emergency Fund is a joint response by the Otto Bremer Foundation and Bremer Bank to the current economic crisis. This growing crisis is creating daunting financial challenges for families and individuals as many struggle to continue to provide food, warm and stable housing, health care, and reliable transportation in the face of increased unemployment and high prices. Timely financial assistance can sometimes make the difference between instability and security, enabling families to meet emergency needs and retain housing or jobs. To help community agencies provide this financial assistance to residents of Bremer communities, the Bremer Emergency Fund will provide grants of up to $100,000 to nonprofit organizations with the capacity to identify community members who can benefit from financial assistance, provide timely disbursements, and offer appropriate follow-up and referral services. The Foundation's grant dollars will be supplemented by a donation from Bremer Bank. The Bremer Emergency Fund provides one-year grants of up to $100,000. Determination of grant size is based on community size and need. Complete proposals must be received by the Foundation no later than December 15, 2008. Proposals received before the December 15 deadline may receive early consideration. Notification of grants will occur by January 15, 2009.
