Brown, MD NEW YORK (Reuters Health) - Disseminating information about fall prevention to primary care providers and their staff and incorporating changes into their practice may help reduce fall-related injuries in elderly patients, according to a report in The New England Journal of Medicine. "This is the first study that attempted to see if clinicians (physicians, nurses, physical and occupational therapists) would be willing and able to incorporate aspects of fall prevention into their practices," lead author Dr. Mary E. Tinetti told Reuters Health, and, if so, whether these practices could reduce serious fall injuries such as hip fractures and head injuries among older adults. The study involved a comparison of fall-related injuries in two regions of Connecticut. Clinicians in one region were took part in the researchers' intervention program, while those in another region did not. "We were somewhat surprised, pleasantly so, that the majority of clinicians were willing to have us come to their site of practice, teach them about fall prevention, and show them how they could incorporate fall risk assessment and management into their practices," noted Tinetti, from Yale School of Medicine in New Haven, Connecticut. Recommended strategies included reducing medications and hazards, treating orthostatic hypotension, in which a change in the body's position, such as standing up, causes a sudden drop of blood pressure, resulting in dizziness or fainting. Other recommendations include correcting visual and foot problems, and training to improve balance, gait and strength. The interventions took place from 2001 to 2004 and their impact was assessed from 2004 to 2006. In the intervention region, 62 percent of primary care offices and 100 percent of home care agencies received an intervention visit. "We were also pleasantly surprised to see that we did reduce the rate of serious fall injuries," Tinetti said. Prior to the interventions, the rates of serious fall-related injuries were comparable in both regions. After the educational program, the rate in the intervention region declined, whereas the rate in the region that did not participate remained largely unchanged. During the study period, the rate of fall-related use of medical services increased in both regions. However, the rise seen in the intervention region was much smaller than that in the usual care region. Clinicians should be aware, Tinetti said, that "there are specific health problems that put older adults at risk of falling (e.g. taking multiple medications, orthostatic hypotension, and unsteady balance or walking). "There are relatively easy ways to identify and improve these problems, and doing so will reduce the likelihood that older adults will suffer fall injuries that can be as serious as heart attacks and strokes."
