Any list serve or staff meeting that you can bring this up to and give me suggestions would be wonderful! Male client, 79, living alone in a co-op setting has lost quite a bit of weight lately. This has seemed to recently stabilize as they found a stomach infection which was treated with medication. We were hired by the family to give medication reminders, provide companionship, encourage eating solid foods more often to put the weight back on and take daily walks. He is pretty unsteady on his feet during the day and often needs to hold onto our Caregiver's arm while walking outside the apartment. He will not use a walker or cane. Family is more concerned about overnight time as he has our Caregivers and his family members checking in on him every morning and every evening. I need ideas to monitor his well being overnight. Because of some memory issues if he does fall any alert buttons will probably not be pushed as he will forget (they are looking for alternative housing ideas as well). The family thought about a camera via computer that can be hooked up, but I don't know how this would monitor him falling at night. Any ideas on what monitoring systems might work when the Lifeline is out of the question? Thank you! The pamphlet states the detector senses a change in orientation and the impact from a fall, then signals to the response center that help is needed. You could check with other lifeline companies also to see if they have this type of service. Tung, MD, Family Physician Description Aging and Adult Services Division is offering a series of videoconferences on roles for community partners in providing quality outcomes for seriously chronically ill and disabled seniors living in the community. Historically, a narrow, medical-centered view of frail seniors has failed to use the valuable "eyes and ears" of community LTC and support service providers to implement more comprehensive chronic illness care. Too often, messages sent to physician clinics about a change of status of a community LTC client have gone unheeded, to the detriment of the senior themselves and to the Medicare budget, with an avoidable crisis, ER visit, and hospitalization result. Physicians counter argue, saying that "unhelpful" information only further hampers their dizzyingly busy offices. Where is the common ground? Learn the perspectives of leading Minnesota geriatricians regarding the opportunities for closer coordination of senior services and medical care. Care coordination, whether formalized by contracts and titles or opportunistic between aging network professionals and individual physicians, needs to be locally grounded and realistic. This session will cover the medical perspective regarding falls with injury and repeated falling. Dr. Tung will include some information about the broad spread efforts at evidence-based falls prevention here in Minnesota, but the primary focus will be on the medical issues and implications for co-existing conditions from falls. Dr. Clouse will share her insights as an urgent care physician responding to fall situations. Discussion and audience questions will help identify specific information's for your community-based work to complement appropriate medical management for falls. Effective strategies to prevent falls have been identified but are underutilized. Methods Using a nonrandomized design, we compared rates of injuries from falls in a region of Connecticut where clinicians had been exposed to interventions to change clinical practice (intervention region) and in a region where clinicians had not been exposed to such interventions (usual-care region). The interventions encouraged primary care clinicians and staff members involved in home care, outpatient rehabilitation, and senior centers to adopt effective risk assessments and strategies for the prevention of falls (e.g., medication reduction and balance and gait training). The outcomes were rates of serious fall-related injuries (hip and other fractures, head injuries, and joint dislocations) and fall-related use of medical services per 1000 person-years among persons who were 70 years of age or older. The interventions occurred from 2001 to 2004, and the evaluations took place from 2004 to 2006. Results Before the interventions, the adjusted rates of serious fall-related injuries (per 1000 person-years) were 31.2 in the usual-care region and 31.9 in the intervention region. During the evaluation period, the adjusted rates were 31.4 and 28.6, respectively (adjusted rate ratio, 0.91; 95% Bayesian credibility interval, 0.88 to 0.94). Between the preintervention period and the evaluation period, the rate of fall-related use of medical services increased from 68.1 to 83.3 per 1000 person-years in the usual-care region and from 70.7 to 74.2 in the intervention region (adjusted rate ratio, 0.89; 95% credibility interval, 0.86 to 0.92). The percentages of clinicians who received intervention visits ranged from 62% (131 of 212 primary care offices) to 100% (26 of 26 home care agencies). Conclusions Dissemination of evidence about fall prevention, coupled with interventions to change clinical practice, may reduce fall-related injuries in elderly persons.
